We welcome all youth 7th - 12th grade to participate in this event - that includes incoming 7th graders :)

I am the parent/legal guardian of the above named youth. The above said youth desires to voluntarily participate in MINI GOLF, BEACH & GAMES DAY, sponsored by Midland Free Methodist Church.
The event is being help on Thursday, July 31, 2025 from 11 am to 9 pm. We will leave the church parking lot at 11 am and head to Bay City's Dutch Village for Mini Golf then on to Bay City State Park for some beach time. We will head to Zef's Coney for dinner and then Crazy Quarters Arcade before returning back to the church by 9 pm.
By typing your name in the field below you assume all responsibility and risk for any accidents, sickness, or other mishaps, including, but not limited to, serious bodily injury, permanent disability, and/or death, that may result to my child on the Program. In connection with my participation in the Program, I hereby waive all rights to any claim, cause of action, and/or the right to file a lawsuit against The Church, or against any of its affiliates, parent organizations, directors, officers, sponsors, employees, agents, volunteers, successors and assigns. I further release all such organizations and/or persons from any and all responsibility or liability of any nature whatsoever for any loss or damage to my person or property, including, but not limited to, personal injury and/or death sustained on or through the Program.
In the event I/we cannot be reached during an Emergency, I the undersigned give permission for my/our child to be treated by a licensed physician, and for said physician to administer whatever care is necesary, including anesthesia, for their safety and care.
